MEDICATION GUIDELINES

UNIFORM GUIDELINES FOR 

THE ADMINISTRATION OF MEDICATION DURING SCHOOL HOURS


The Wissahickon School  District recognizes that parents/guardians have the primary responsibility for the health of their children.  Although the District strongly recommends that medication be given in the home, it realizes that the health of some children requires that they receive medication while in school.  We urge the cooperation of all parents/guardians when it is necessary for a child to receive medication in school.  Parents should confer with the child's physician to arrange medication time intervals to avoid school hours whenever possible. 

 
When medication absolutely must be given during school hours, 

certain procedures must be followed:  

FOR PRESCRIPTION MEDICATION

1. The parent/guardian must give written consent for prescription medication.

2. Any medication to be given during school hours must be delivered directly to the School Nurse.  The medication must be brought to school in the original pharmaceutical-dispensed and properly labeled container.

3. In the absence of the School Nurse, the building Principal or his/her designee will administer the medications.

4. Students in grades 6 through 12 will be responsible for reporting to the Nurse at the time the medication is to be given.  In grades K through 5, individualized plans will be made for the administration by the Nurse, the Principal or his/her designee.

5. Prescription medication for more than two weeks must be considered long term.  A long term request form must be completed by the physician before medication will be given.  Contact School Nurse for form.

6. Students requiring prescription inhalers must obtain a request form to be completed by parent/guardian in order to carry for self administration. Contact School Nurse for form.

7. If there is a change in medication or dosage, rules # 1,2,4 & 5 as set forth above must again be adhered to.

FOR NON-PRESCRIPTION MEDICATION

1.  Medication short-form is required.  Obtain from School Nurse.   Or a note from parent/guardian giving the following information:
      a. Name of  Student, Grade, Teacher
      b. Name of Medication
      c. Dosage (amount to be given)
      d. Why the medication is to be given
      e. The time medication should be given

2. Note will be taken by the student to the Nurse or, in her absence, to the building Principal or his/her designee at the beginning of the school day.  The medication must be brought to school in the container in which it was purchased.  This applies to all students.

3. Students in grades 6 through 12 will be responsible for reporting to the Nurse's office at the time the medication is to be taken.  In grades K through 5, individualized plans will be made for the administration of medication by the Nurse, Principal or his/her designee.

FOR CONTROLLED SUBSTANCES

Controlled substances, such as Ritalin; Adderall; Codeine, MUST be brought to school by a parent/guardian.  If it is a long-term medication, REFILLS MUST ALSO be brought by a parent/guardian.
